Chart Analyst Health Information Medical Records SCH ID-2633
Summary:
The Chart Analyst will review each discharged patient's record for compliance with documentation requirements established by regulatory agencies and medical staff rules and regulations. Review system-assigned deficiencies for appropriate allocation; investigate and reassign declined deficiencies returned to the various pools and assist providers in completing the medical records electronically and in a timely manner.

Responsibilities:
EDUCATION
- High school graduate or equivalent required
- Associate degree in health related field required; AS in Health Information Technology with certification as a RHIT is preferred
- In lieu of an Associates degree, a candidate with three-five (3-5) years of Health Information department experience and medical terminology may be considered
EXPERIENCE
- Six (6) months experience in a Health Information department is preferred
KNOWLEDGE, SKILL AND ABILITY
- Computer literate
- Medical terminology required, must successfully pass test
- Visual acuity to read physician documentation in the medical record to distinguish completeness
- Ability to read medical record numbers and patient names
- Ability to utilize the computer to enter the physician deficiencies throughout the day
- Strong analytical and organizational skills, ability to prioritize workloads and meet deadlines
- Work effectively under pressure while maintaining excellent customer service skills
- Familiar with basic medical terminology; knowledgeable of medical record regulatory requirements as well as medical staff rules and regulations
WORKING CONDITIONS
- Individual works in an office setting using medical records and computers to perform duties
- Occasional travel to department storage areas and incomplete record filing area to retrieve medical record
ENVIRONMENT
- Normal heat, light space, and safe working environment; typical of most office jobs
